A ticking time bomb: Abdominal Aortic Aneurysm

Case: 838175

- 81 y/o male


Clinical Presentation:

- Unstable, peri arrest

- Bleeding intra-abdominal

- Intubated, on high dose pressors

- ? dissection of AAA


Previous History:

- Previous smoker

- Coronary artery disease


Diagnosis

- Ruptured infrarenal abdominal aortic aneurysm


Imaging Performed:

- CT angiogram of the thoracic and abdominal aorta with intravenous contrast







Report Findings:

- Ruptured fusiform infrarenal abdominal aortic aneurysm measuring 6.3 (AP) x 6.8 (ML) x 9.8 (SI) cm with active contrast extravasation

- Large retroperitoneal haematoma with positive mass effect displaces and partially surrounds the right kidney with resultant occlusion of the IVC, and severe compression of the right renal and portal vein.


Treatment Plan:

Palliation (Patient sadly passed away a few hours later)
